Text
Upon request by a person conducting an investigation under this chapter, a school, day care center, child care facility, residential facility, residential treatment facility, or similar institution shall provide the person conducting the investigation with:
(1)The name, date of birth, Social Security number, and last known address and phone number of any person identified as an alleged offender if the alleged child maltreatment occurred at that school, center, or facility; and (2) The name and address of any witness to the alleged child maltreatment if the alleged child maltreatment occurred at that school, center, or facility.
