Text

(a)A gift of all or part of the body may be made by the holder of a valid Alabama Driver’s License or nondriver identification card by the execution of a sworn affidavit to be filed with the Alabama State Law Enforcement Agency.
(b)Notice of intent to make a gift shall be noted on the driver’s license or nondriver identification card of the donor in a manner to be determined by the Alabama State Law Enforcement Agency and there shall also be noted thereon the specific gift of the donor in accordance with the following legend: E - Eye K - Kidney H - Heart Li - Liver L - Lungs A - All (everything)
(c)The gift shall become effective on the death of the donor without any formal requirements of delivery.
(d)The affidavit shall be signed by the holder of the driver’s license or nondriver ide
