Text

The form of the affidavit which shall be printed on the envelope used in primary elections shall be the same as that used in general, special, and municipal elections; except, that the following sentence shall be added and inserted therein immediately above the signature of the voter: “I am a member of the ________ Party and subscribe to all conditions and qualifications laid down by that party’s committee as a requisite for participation in the election ________ (Signature of Voter).” Such affidavit shall be sworn to in the same manner and form as in general, special, and municipal elections.
