(a)LEGIBILITY; ABBREVIATIONS. A petition, pleading, schedule, or other document must be clearly legible. Commonly used English abbreviations are acceptable.
(b)CAPTION. A document presented for filing must contain a cap- tion that sets forth:
(1)the court’s name;
(2)the case’s title;
(3)the case number and, if appropriate, adversary-proceed- ing number; and
(4)a brief designation of the document’s character.
