Text
(a)CAPTION AND TITLE; REQUIRED INFORMATION. A petition’s cap-
tion must contain the name of the court, the title of the case, and
the case number (if known). The title must include the following
information about the debtor:
(1)name;
(2)employer-identification number;
(3)the last 4 digits of the social-security number or individ-
ual taxpayer-identification number;
(4)any other federal taxpayer-identification number; and
(5)all other names the debtor has used within 8 years before
the petition was filed.
(b)PETITION NOT FILED BY THE DEBTOR. A petition not filed by
the debtor must include all names that the petitioner knows have
been used by the debtor.
